Get Started with Item to Form
Item to Form lets you turn any monday.com item into a shareable form in seconds. Follow this guide to install the app, create your first form, and share it with respondents.
Prerequisites
- A monday.com account with at least one board containing items
- Admin or editor permissions on the board where you want to create forms
Step 1: Install from the Marketplace
Open the monday.com marketplace and search for "Item to Form". Click Install and authorize the app to access your workspace.
monday.com → Apps Marketplace → Search "Item to Form" → Install → AuthorizeOnce installed, the app becomes available as an item view on every board in your workspace.
Step 2: Open the App
Navigate to the board and item you want to create a form for. Open the item and look for the Item to Form view tab. Clicking it launches the form generation interface.
Step 3: Select Columns
The app reads all supported columns from your board and displays them as selectable pills. Choose which columns should appear as fields on the form.
- Use Select All or Clear All to quickly toggle every column
- Click individual column pills to include or exclude them
- The selection count updates in real-time so you always know how many fields are included
Item to Form supports 14+ column types including text, numbers, dropdowns, status, date, email, phone, URL, checkbox, and people columns. See Supported Field Types for the full list.
Configure People Columns
If your form includes a People column, you must configure which team members will be selectable by form respondents. The app fetches all available people from your board and lets you pick a subset.
Step 4: Generate and Share
Click Generate to create the form. The app will produce a unique shareable URL that you can copy to your clipboard with one click. Share this link with anyone — no monday.com account is required to fill out the form.
- Click the Generate button
- Copy the shareable link from the confirmation screen
- Send the link to respondents via email, chat, or any channel
Submitted responses are automatically synced back to your monday.com board item.
